Invasive mechanical ventilation represents a method of respiratory support that seeks to maintain the patient until they are able to breathe independently, and the weaning process consists of the gradual withdrawal of ventilatory support. The objective of the present work was to discuss the relationship between peripheral muscle strength and weaning from mechanical ventilation, highlighting the importance of the physiotherapist in this context. Peripheral muscle strength is related to weaning from mechanical ventilation in ICU patients. Muscle weakness acquired during intensive hospitalization can make the weaning process difficult, prolonging the need for ventilatory support and this occurs because muscle weakness compromises the patient's ability to perform the respiratory efforts necessary to breathe spontaneously, contributing to complications such as the inability to cough effectively and maintain adequate ventilation. Inspiratory muscle training increases maximum inspiratory pressure, improving weaning success rates and reducing the time required to completely withdraw ventilation. Used safely and supervised, training offers benefits such as reduced hospital stays. Clinical assessments can identify patients with the greatest potential for improvement, especially those with previous weaning failures. Other strategies, such as abdominal functional electrical stimulation and early reduction of blood pressure support, have also shown positive results, but there is no consensus on the most effective methods. Muscular rehabilitation, including physiotherapy and early mobilization, can contribute to the recovery of peripheral muscle strength, improving the chances of successful weaning from mechanical ventilation and the patient's general recovery.
