Introduction: Rheumatologic diseases encompass a spectrum of chronic conditions with multifactorial causes associated with inflammatory processes. With an estimated prevalence of 2.2% to 3.0% in the population, these conditions require effective therapeutic approaches. In vivo and in vitro studies have highlighted the potential of probiotics as reducers of systemic inflammation in rheumatologic diseases. They act through immunomodulatory effects, improving immune system function, intestinal mucosal barrier integrity, correcting intestinal microbiological disturbances, promoting the secretion of anti-inflammatory factors, and inhibiting the growth of harmful bacteria. Consequently, they have been investigated as potential therapeutic options for various inflammatory conditions such as psoriasis, fibromyalgia, rheumatoid arthritis, spondyloarthritis, juvenile idiopathic arthritis, and systemic sclerosis. Objective: To investigate the efficacy of probiotics in treating rheumatologic conditions. Methodology: A systematic umbrella review of systematic reviews was conducted using electronic databases such as PubMed, BVS, SciELO, ScienceDirect, and Cochrane, following PRISMA guidelines. Methodological evaluation was performed using AMSTAR-2. Results: A total of 1,652 studies were identified with most excluded after applying eligibility criteria and removing duplicates, resulting in 12 systematic reviews included. These reviews, published between 2017 and 2024, focused on the use of probiotics in treating autoimmune and inflammatory diseases, primarily analyzing randomized clinical trials with variations in sample sizes and methods of analysis such as DAS28 and CRP. Probiotics may reduce inflammatory markers and improve disease activity, particularly in rheumatoid arthritis, although some studies reported mixed results. The quality of evidence and risk of bias ranged from low to high. Mild adverse events were reported. Conclusion: Although probiotics demonstrate therapeutic potential in rheumatologic conditions, current evidence remains insufficient to justify their routine clinical use.
